In the state of Arizona, apostilles are issued by Ken Bennett, the Secretary of State in Phoenix, Arizona.
Documents submitted for apostille must comply with the following guidelines:
- Documents such as birth, death or marriage certificates considered vital records must be certified by the current State registrar, Patricia Adams and issued no earlier than 5 years ago.
- Notarized documents must be certified by Arizona County Clerk and properly notarized with a notarial jurat certificate.
- Academic documents need to be notarized by the school registrar.
